Men's Extended Care Substance Abuse Treatment

Following the completion of traditional inpatient treatment programs, people recovering from a substance use disorder might require support to increase their chances of lifelong recovery. Extended Care programs provide the counseling, education, and wellness activities you need to learn how to live a life free of substance use. The Men's Extended Care Program is a 60-day program for emerging adult males that builds off of the Emerging Adult Primary Care Program. Individualized treatment plans, recreational activities, life skills training, and counseling motivate young men to take back control of their lives.

NESTLED INTO THE CHESAPEAKE BAY, WE DESIGNED OUR FACILITIES TO GIVE OUR PATIENTS THE MOTIVATION NEEDED TO HEAL THEIR MIND, BODY, AND SPIRIT. WE TAILOR OUR INPATIENT PROGRAMS TO EACH INDIVIDUAL'S NEEDS AND USE MEDICAL, CLINICAL, AND HOLISTIC CARE TO HEAL THE WHOLE SELF. WITH EVIDENCED-BASED TREATMENT, WE'LL HELP TO INCREASE THE CHANCES OF LONG-TERM RECOVERY SO PATIENTS CAN LIVE LIFE FULLY AGAIN. EACH PATIENT WILL RECEIVE AN ASSESSMENT UPON INTAKE, ONSITE WITHDRAWAL MANAGEMENT, THERAPEUTIC SERVICES, SKILLS TRAINING, AND MENTAL HEALTH SERVICES. ALL OF THESE PLAY A ROLE IN OUR ABILITY TO FIND AND TREAT THE ROOT CAUSE OF SUBSTANCE USE DISORDERS FOR EACH PATIENT THAT ENTERS OUR CARE. WE OFFER THE FOLLOWING PROGRAMS: PRIMARY, RELAPSE, EMERGING ADULT, AND PAIN RECOVERY.

Inclusions

The Bundled Rate plus Outlier Per Diems includes:

1. A 60-day base program running Monday through Sunday for adults who have successfully completed inpatient treatment and would benefit from additional treatment. 2. Medical Management (MAT & OMT), substance use disorder education, individual and group counseling, life-skills training, case management. 3. Family therapy services, nature and therapeutic art activities, spiritual counseling, nutrition and exercise plans, recreation activities, peer recovery support and discharge planning. 4. Recovery Support Groups (AA, NA, SMART) and support obtaining a sponsor. 5. Therapeutic Sessions include 1 to 2 hours of Individual sessions weekly, 10 hours of Group therapy sessions weekly, and 5 hours “life skills” group weekly sessions. 6. Daily meals, laundry services (excluding dry cleaning). 7. In the event the patient re-admits to treatment within 30 days of the original episode of care or within the Bundle Payment Period, the subsequent readmission is included in the bundled rate payment and subject to the LOS threshold and outlier per diem. The Bundled Rate period begins on the day of the procedure for the scheduled Accarent Procedure and covers up to 60 inpatient days in the hospital. Should the length of stay exceed 60 inpatient days, then per diem outlier payments will be applicable for each subsequent inpatient day. The bundled rate period ends at discharge.

Exclusions

Medications (over the counter and prescription, including specialty). Housing is a component of the Extended Care Program however, housing is not included in the bundled rate. Laboratory studies and toxicology screening (billed to patient's insurance). Postage fees. Should a patient leave the treatment program early or discontinue treatment against medical advice, the total bundled payment amount will still be due.

Candidacy

Candidacy for the bundled procedure will be determined by the Provider.
