The Bundled Rate plus Outlier Per Diems includes:
1. Pre physical health risk assessment.
2. Comprehensive physical examination with an Internal Medicine specialist.
3. Comprehensive laboratory testing, which include a blood count (with differential), lipid panel, metabolic panel (including blood glucose), HIV, TSH, urinalysis, iron studies, HbA1c, Hep C, and prostate-specific antigen (PSA).
4. Vaccination review and administration as needed.
5. Hearing evaluation with screening audiogram.
6. Visual health evaluation with an Optometrist, which will include fundus photography, visual acuity test and tonometry screening.
7. Skin health exam with a Dermatologist.
8. Cardiovascular assessment, which will include a coronary calcium CT scan or, if indicated cardiac stress test, electrocardiogram (ECG), carotid & abdominal aortic ultrasound, and doppler flow peripheral vascular index testing.
9. Pulmonary health evaluation with spirometry and chest X-ray (CXR) / low-dose CT.
10. Cancer screenings including Cervical and Breast cancer for women and Prostate cancer screening for men.
11. DEXA/Bone density scan.
12. Personalized dietary consultation with a registered dietitian.
13. Stress-management training.
The Bundled Rate Period for the Covered Services begins the day of the procedure and ends after the PATIENT's same day discharge from the Bundled Procedure.
