GALLERI IS RECOMMENDED FOR USE IN ADULTS AT ELEVATED RISK FOR CANCER, SUCH AS THOSE AGED 50+. THE GALLERI TEST DOES NOT DETECT A SIGNAL FOR ALL CANCERS AND NOT ALL CANCERS CAN BE DETECTED IN THE BLOOD. FALSE POSITIVE AND FALSE NEGATIVE SIGNAL RESULTS DO OCCUR.

Inclusions

The Bundled Rate plus Outlier Per Diems includes:

1. Telemedicine provider for patient consultation, eligibility check, and test ordering. 2. Shipment of test kit to the patient's home and facilitation of blood draw at home or through a lab partner. 3. Delivery of result, including expert peer-to-peer primary care physician (PCP) support for “cancer signal detected” result. 4. GRAIL patient advocate supports patients who receive a “cancer signal detected” result. 5. Customer Service hotline for patient and provider support throughout the testing process. The Bundled Rate period begins with the initiation of bundled services and ends when the included services have been completed.

Exclusions

Use of Galleri is not recommended in individuals who are pregnant, 21 years old or younger, or undergoing active cancer treatment.

Candidacy

Candidacy for the bundled procedure will be determined by the Provider.
