Proton Therapy For Pediatric Patients

Inclusions

The Bundled Rate plus Outlier Per Diems includes:

1. The Bundled Rate is a daily Proton Fraction fee which includes facility services, supplies, access to equipment and professional services needed to delivery the Proton Fraction. 2. The Bundled Fee is inclusive of Daily Proton Treatment services which include the planning, simulation, immobilization devices, CT imaging services and the one proton delivery fraction. 3. If more than one proton fraction delivery is required, then the associated daily fraction fee will apply (found in the Letter of Agreement) in addition to the above bundled rate. 4. If the proton equipment is off-line for maintenance, then daily photon radiation will be delivered as substitutes at the above bundled rate. The Bundled period for the procedure begins on the date of treatment planning and lasts for 90 days, ending no sooner than 30 days following the last day of prescribed radiation.

Candidacy for the bundled procedure will be determined by the Provider.
