Inclusions

The Bundled Rate plus Outlier Per Diems includes:

1. All enrolled participants receive a Digital Therapist kit that assists them in performing their exercises, provides video and audio descriptions of each exercise, and provides real-time biofeedback during exercise execution. 2. The Digital Therapist kit uses sensor data to make clinical decisions based on member needs. 3. Participants can download the mobile app to their phones and create a user profile, receive educational articles on MSK health and behavioral advice to support wellness, and communicate directly with their specialist through the mobile app. 4. Participants will receive Sword’s FDA-listed, wearable inertial sensors that guide them in their exercise, correct their form, and collect data. The bundled rate covers services from registration through 365 days.
