Inclusions

The Bundled Rate plus Outlier Per Diems includes:

1. A 2-hour diagnostic evaluation. 2. The initiation of specialized behavior therapy for underweight anorexia nervosa or avoidant restrictive food intake disorder. 3. Treatment consists of no more than six (6) hour-long Mental Health Treatment Sessions. 4. Given the complexities of individual cases, additional therapy sessions can be purchased to further support patient care at the Additional Hourly Rate at the above per diem rate. The Bundled Rate period begins with the initiation of bundled services and ends when the included services have been completed.

Exclusions

Additional diagnostic services consultation, or evaluation services are Excluded from the Bundled Rate, and are payable at the Default Rate when services are rendered outside of the Bundled Rate Payment Period in relation to the Comprehensive Mental Health Assessment service.

Candidacy

Candidacy for the bundled procedure will be determined by the Provider.
