Johns Hopkins Work Stride

The Bundled Rate plus Outlier Per Diems includes:

1. 365 days of unlimited access to the Work Stride website for the patient, family, employer, and caregiver(s). 2. Website access provides information and support regarding prevention and screening, diagnosis, health plan benefits, government entitlements, and how to manage work while undergoing treatment or caring for a loved one with cancer. 3. Up to eight (8) 30-minute consultations with a Work Stride Oncology Nurse Navigator to help manage work-related, financial, and emotional aspects of dealing with a cancer diagnosis and treatment. 4. The Nurse Navigators are available to guide members to the right care, at the right place, and at the right time and assist with making the most of tools and resources available. The bundled rate covers services from registration through 365 days.

Candidacy for the bundled procedure will be determined by the Provider.
